Athletic Forms - Physicals & Final Forms Information
Physical Form
A current-year physical must be given on or after April 15 of the previous school year. All physical forms must be turned in to the Athletic Office before the first day of try-out.
Final Forms
All athletes are required to have a Final Forms account. In order to be eligible to practice and/or compete, all forms must be completed and electronically signed by parents and athletes, physical forms, and pay to participate fees must be turned into the Athletic Office.
